Don’t be overwhelmed by the number of forms in this packet!! Below you will find an easy guide to understanding what you have. Make sure that you and your parents read everything in the packet carefully.
- Candidate letter- This is your official notice of continuing eligibility to participate in the National Honor Society contingent on second semester junior grades.
- NHS Senior Update- You must fill this out and return it with a $30.00 check to continue your candidacy in NHS. Make sure that your parents sign the bottom before you turn it in!
- NHS statement- This describes what the National Honor Society is about and it gives you an overview of the requirements for our members and candidates.
- Frequently Asked Questions- This is a summary that we have compiled over the years to answer the most common questions of candidates and their parents. Read this carefully to make sure you understand all aspects of your NHS candidacy.
- Community Service Instructions- This is a step-by-step instruction sheet that details how to begin and complete your service hours.
- NHS Approved Agency List- This is a listing of those agencies that have worked with our volunteers before. You can choose to do service from any agency on this list.
- Outside Agency Contract- Use this form if you want to do service work for an agency that has not been pre-approved. You can discard this form if you are choosing from the Approved Agency List.
- Agency Contract- This is what you and the agency of your choice must sign before your begin your hours. Make sure that your parents sign as well before you turn in to your adviser.
- Volunteer Plan- This form will help you plan out your volunteer hours for next year. You need to sign the form along with your parents and the agency. It must be turned in with the Agency Contract.
- Hours Log- This is where you record each time that you complete service hours.
- List of important dates- Keep this list handy!! Make sure that you clear specific dates and plan ahead for meetings and Unity Projects.
- Unity Project Form- Complete and turn in this form after you finish your Unity Project.
- Meeting Policy- This explains the NHS requirements for attending mandatory meetings. READ THIS CAREFULLY.
